Answer:
The « form of a horse » is a summary of the past performances of a horse. This is a row of letters and numbers which need to be read from left to right. The left performance is the most recent one. The numbers (0 to 9) stand for the placing of the horse at the arrival. The letters stand for the discipline.

How to read the example?
The horse ran twice in 2014: the last time he won the race (harness) and the second time he was disqualified. In 2013 he ran only twice and was disqualified both times.
Number from 1 to 9: position of the horse at the arrival
Number 0: means that the horse was not placed in the first ten horses at the arrival
Two following numbers mean a change of year. In the example above the performances on the left of “13” happened in 2014, on the right of “13”, in 2013.

How to read the example?
The horse ran twice in 2014: the last time he was placed 7th in a hurdle race and the previous time he won the race. In 2013 he ran only twice in flat racing: once he was placed 6th and the time before 2nd.
